Chapter 16: Patients with Diabetic Nephropathy in Established Renal Failure: Demographics, Survival and Biochemical Variables

چکیده
Diabetic nephropathy is now the most common renal disease leading to renal replacement therapy in developed countries. Within the UK, the number of DN patients accepted for RRT rose steadily in the 1990s especially in the African–Caribbean and South Asian populations. This may be related to the increased prevalence of Type 2 diabetes in the general population, the ageing population and the liberalisation of attitudes to acceptance for RRT. The overall rise has slowed in the last 4 years. DN patients starting RRT are likely to have more co-morbidity than other patients, in particular cardiovascular disease, and consequently worse survival on RRT. In recent years there has been some reduction in the high mortality of such patients, so the prevalence of diabetic nephropathy patients on RRT (currently lower than the percentage of incident patients, see Chapter 3) might increase.

Comparison of Diagnostic Value of Intrarenal Doppler Indices and Microalbuminuria for Detection of Diabetic Nephropathy in Type II Diabetic Patients
Background & Aims: Diabetes is the most common cause of renal insufficiency throughout the world. The main cause of renal failure in diabetic patients is microvascular endothelial injuries that is secondary to atherosclerosis and ischemia. The efficacy of renal Doppler indices such as resistive index (RI) and pulsetile index (PI) for the evaluation of intrarenal arteries has been shown. This st...
متن کاملRisk Factors for Diabetic Nephropathy in Diabetic Patients
Aims: Diabetic nephropathy is one of the consequences of type 1 and type 2 diabetes and one of the main causes of End-Stage Renal Disease (ESRD) as well as an important risk factor for cardiovascular morbidity and death. The aim of this study was to identify the risk factors for diabetic nephropathy in diabetic patients. Instruments & Methods: In this cross-sectional descriptive study, 121 pat...
متن کاملOutcome of renal transplantation in patients with type 2 diabetic nephropathy: a single-center experience.
BACKGROUND Renal transplantation has been established as a treatment for end-stage renal disease (ESRD) due to diabetic nephropathy. However, few studies have focused on the outcome after renal transplantation in patients with ESRD and type 2 diabetic nephropathy. To investigate the effect of renal transplantation on ESRD with type 2 diabetic nephropathy, we retrospectively analyzed patients wh...
متن کاملRelationship between Serum Homocysteine and Other Parameters in Overt Diabetic Nephropathy
OBJECTIVE: Level of serum homocysteine is remarkably common among patients with moderate to sever renal failure. We assessed whether serum homocysteine concentrations were correlated with proteinuria in overt diabetic nephropathy. MATERIALS AND METHODS: A cross-sectional study was carried out on 45 patients (29 males, 16 females) with overt proteinuria. All subjects had type 2 diabetes. Data i...
متن کاملRelationship between Intra-renal Arterial Resistance Index (RI) and Albuminuria in Diabetic Patients
OBJECTIVE: Diabetic nephropathy is a major diabetes complication. Arterial resistance index (RI) may predict deterioration in kidney function. This study was designed to compare renal arterial RI in different stages of renal function according to glomerular filtration rate (GFR), serum creatinine level and proteinuria. MATERIALS AND METHODS: In a cross-sectional study on 81 diabetic patients i...
